Student Vaccination Distribution Interest Form Ages 12+

The Newburgh Enlarged City School District is planning to host a vaccine distribution in the upcoming weeks. Please complete this form if you are interested in receiving the vaccine. Once details are finalized, you will receive an email from Communications@necsd.net and a phone call from the district (845-563-3400). Please make sure these contacts are in your contact list, so you do not miss out on important information.

Scholars who are interested can share this form with family members if they would like to receive the vaccine together.
