Information about Upcoming School Nurse Report

Attention families of scholars in grades: Kindergarten, 1st, 3rd, 5th, 8th and 11th 

Our school nurses will be reporting to the New York State Department of Health regarding student weight status category (based on  BMI percentile). The information is collected  from the mandated health examination forms submitted during School year 2017-2018 school year physical exam and no names are connected or part of the reporting. If you would like to opt-out for your child, please complete the form that can be found below and return it to your school nurse no later than Friday, January 18th. Opt-out forms can also be found by visiting or calling the nurses office in your child's school.
